About avoiding the pitfalls of remote work from home

After almost 10 years of remote work - mostly from home - I figured out the factors that can cripple my productivity, sway my life-work balance into the wrong direction and lead me towards a couch potato lifestyle. What are those pitfalls and how am I trying to avoid them? And most importantly, is it working?
